Statutory Declaration for Alberta Proof of Identity

If you reside in Vancouver, and need to file a Proof of Identity application for the Alberta Vital Statistics documentation services you will need to submit the necessary Statutory Declaration. The form is called DVS3317, and it can be sworn at Brosgall Legal in Vancouver. This Statutory Declaration ensures that you submit acceptable and original Proof of Identity documnet to make your application to Alberta Vital Statistics. The Statutory Declaration also asks you to list the document that proves your identy (such as a passport), where it was issued, your name, that it contains a photo, if it expires or not, and the identificaton number on your Proof of Identity document. Finally, you will declare that you make the declaration conscientiously believing it to be true, as if you made it under oath.

A statutory declaration is a written document of facts that the declarant (you) says is true. The Alberta Vital Statistics Agency provides the necessary form (DVS3317). Brosgall Legal then notarizes the declaration with our signature, stamp and seal.

Certified Copies

A certified copy is a photocopy of a document that a lawyer or notary compares to the original, and then certifies to be a true copy of that original by applying their signature and seal on the copy.
