Statutory Declaration for BC Change of Name Applications

Under the provisions of the British Columbia Name Act, applicants who want to change their name must submit proper documentation. One of the requirements is a Statutory Declaration .This Statutory Declaration ensures that you have read the application and to the best of your knowledge, information and belief, the statements made within it are true in substance and in fact. You will also need to delare that you understand the submitted documentation, and that you have enclosed the necessary certificates.  Finally, you will declare that you understand that all previously issued birth certificates and BC marriage certificates will not be returned to you on the completion of the name change.

Statutory Declarations

A statutory declaration is a written document of facts that the declarant (you) says is true. The BC Vital Statistics Agency provides the necessary form on page 6 of the application package. Brosgall Legal then notarizes the declaration with our signature, stamp and seal.

Certified Copies

A certified copy is a photocopy of a document that a lawyer or notary compares to the original, and then certifies to be a true copy of that original by applying their signature and seal on the copy.
